Student Dental Plan Enrollment Application
• This plan is available only in states where Savon has Network Preferred or Network Participating Providers.
• Membership begins on the day you join (spring, summer or fall semester) and expires in 12 months.
• You must be a college/trade school student (full or part time), between the ages of 18-30 and proof of enrollment may be required.
