To Your Health With Jourdin Hendershot:

Blepharoplasty

Jourdin
Are you noticing a drooping of your eyelids?
Do you feel like they make you look more tired than you feel?
Are your eyelids affecting your vision?
Do you feel you look older than you are?

If you answered yes to any of these questions, you should schedule a visit with an optometrist to discuss treatment options.

Have you ever wondered what causes drooping eyelids?– Well, many factors can play a role in eyelids drooping.

The three main reasons are: 1.  Stretched tendons or weakness of the muscle that raises the eyelids.  2.  Damage to the nerves that control those muscles.  3.  Looseness of the skin on the upper eyelids.

On the other hand, it could be caused by the natural aging process.  As you age, the eyelids can stretch, muscles weaken, and fat can begin to gather around the eyes.  This can cause sagging eyebrows and droopiness of the eyelids and those dreaded under–eye bags.

Luckily there is an eyelid surgery called Blepharoplasty.  This is typically, an outpatient procedure that removes any excess tissue.  You may be given injections into the eyelids to numb the area (this will all depend on the surgeon).  The surgery usually takes about an hour but in difficult cases, could take longer.

To correct the upper eyelid, the surgeon will cut along the fold of the eyelid.  For the lower eyelids, the surgeon cuts just below the lash line.  Once the cuts are made, they will remove the excess skin, muscle, and possible fat and the incision will be closed.

After surgery, it‘s normal to experience:
  • Blurred vision
  • Watery eyes
  • Swelling and bruising
  • Pain or discomfort
It‘s important to follow the after–care instructions provided by your doctor.

Now comes the question…– who pays for this?  Well, this can be tricky.

If sagging skin interferes with your vision and the doctor determines that the surgery is a medical necessity, your insurance may pay for the procedure.

If it is not a medical necessity, and the surgery is being done for cosmetic reasons, most insurances will not pay for it.

Before moving forward with this procedure, it‘s best to consult with the optometrist and your medical insurance company!

Here’s Your Answer

Questions From Our Members

R. Richards of Fresno, California asks:

“Prior to making an appointment with one of your providers, I checked them out for complaints with the dental board.  This particular dentist (I'm not going to mention their name), had several complaints.  Does Savon know about these complaints and if so, why is the dentist on your plan?”

Savon’s Answer

Unless a complaint is recent (within the last 90 days), the answer is YES, we do know about any complaint that any of our network providers have.

The reason that we have them on our plan is because the severity of their complaints does not raise an issue with our credentialing department staff, one of which is a retired dentist.

The bottom line is no dentist is going to be able to please everyone they treat.  Some patients file a complaint with the board without even trying to resolve the problem with the doctor.  Some patients are always looking for anything that they can get for free.

That being said, we check all of our network providers for complaints every four months.  If we determine a complaint is serious enough, we take the proper actions with the most drastic being revocation of their certification with Savon and removal from the plan.

Prednisolone May Cause Delayed Healing After Tooth Extraction

A direct reprint of an article by Alex Dagostino, DrBicuspid.com associate editor

Patients taking prednisolone following a tooth extraction may experience delayed healing of the extraction socket.  A study published on September 8 in the Journal of Dental Sciences identified specific dosages of prednisolone and other factors associated with delayed healing of the extraction site.

The study included 80 patients who had a tooth extracted under local anesthesia and were taking prednisolone orally.  Patients were divided into groups based on their healing, and their background and dosage of prednisolone were compared.

The dosage of prednisolone was significantly higher in the delayed healing group compared to the nondelayed healing group.  The main factors for delayed healing included the dosage of prednisolone taken and osteosclerotic changes around the extraction socket.
“In this study, the cutoff dosage of 8.0 mg/day of prednisolone was determined as the preoperative dosage that causes delayed healing,” wrote the study authors, led by Hiroyuki Hato of Hokkaido University in Japan.  Exceeding this dosage may increase adverse side effects and cause healing failure, they wrote.

The wound–healing process can be divided into three phases: the inflammatory phase, the proliferative phase, and the remodeling phase.  At each phase, prednisolone regulates the expression of cytokines and the migration of immune cells, causing delayed wound healing.

Osteosclerotic changes in the maxilla and mandible arise from infection, causing inflammation and potentially resulting in nutrient deprivation and delayed healing.  In the present study, osteosclerotic changes beyond the alveolar bone near the extracted tooth were significantly associated with delayed healing.

“Long–term chronic inflammation of osteosclerotic changes can lead to osteomyelitis beyond the alveolar bone,“Hato and colleagues wrote.

The present study focused on wound healing and preoperative antibacterial agents as factors that prevent delayed healing following tooth extraction in patients taking prednisolone orally.  Neither of these factors was independent in delaying healing.

“Careful attention to dosage of prednisolone and findings of radiographs when performing tooth extraction is important to prevent delayed healing,” Hato and colleagues concluded.

Until next time; brush, floss and keep smiling!

A Holiday Message From Our Family To Yours

Wow!  Another year has already flown by!  Time flies when you are having fun!  We want to take this opportunity to say thank you to our members who have continued to show their trust by keeping their dental plan.  We would like to say a special thank you to all of our members that have referred new members to us.  It is only because of our members that we have been able to survive these tough economic times.

Here at Savon, we are doing everything in our power to keep from adjusting the Schedule of Fees and Benefits (what you pay at the dentist).  We will continue our efforts to every extent that we can until we start losing providers.  We are in constant discussions with our providers and we are trying to get them to understand that even in these inflationary times, raising our fees is not in the best interest of our members, our plan and their practice.

With that in mind, certain adjustments are going to need to be made, however rest assured we will do our best to keep the adjustments as small as possible.

As we celebrate the holidays we would like to remind you of our holiday hours:

December 2022
  • Closed Friday December 23, 2022 through Monday December 26, 2022 for Christmas
(We will re-open on Tuesday the 27th)
January 2023
  • Closed Friday December 30, 2022 through Monday January 2, 2023 for New Years
(We will re-open on Tuesday the 3rd)
From our family to your family, we wish you a happy and safe holiday season and a prosperous 2023!
