Gift Certificate Purchasing Instructions

State the reciepent lives in
Select a Dentist Use default of 5105
(This is only for processing and will change when plan is activated)
Select type of plan - Single, Double or Family
Name of the reciepent
Purchaser's billing address
Purchaser's telephone number
Birthdate and age of the reciepient
Last 4 of SSN: Use default of 1234
Purchasers email address (for receipt purposes only)
Under depentents Use default of 0

Click on the "Place your gift card order today" button

Complete the Applicaion using the information from line 1 above

Members - input the code GIFT plus your plan ID number in the promo code box. Non-members use GIFT plus your phone number. ie: (GIFT12345) or (GIFT6025551212)

Your credit card will be charged for the correct amount and a gift certificate will be sent to you
