Cohen’s Corner – Straight talk from the Doctor:
The “Standard of Care” What is it?
This month I will be discussing the standard of care: What it is and why it is used. The standard of care is a term used by regulatory agencies as a yardstick to measure health care quality and the question of whether this care is within an acceptable range or has fallen into an unacceptable range by the local community. You may think of the standard of care as a type of grading system for health care issues. The standard of care is usually based on what and how a prudent health care provider in the same community would handle a similar situation. Let me give you some examples of this concept in the dental field.
If the standard of care for a silver filling is defined in my neighborhood as it was when I was in dental school: that a silver filling should not be more than 1/3 the width of the whole tooth, then a silver filling placed in a tooth that is more than 1/3 the width of the tooth is below the standard of care in the community. In this example the tooth needed more coverage with an onlay or crown to protect the tooth from fracturing.
Another example of the standard of care concept in the dental field is if a tooth needs a root canal, post, build-up, and crown to restore it but this tooth has a very large amount of bone loss around it then the question arises as to what to do under these circumstances. Sometimes when the general dentist can’t figure out what the standard of care is and can’t make a definitive treatment plan, he will then ask the specialist to evaluate the situation and to determine what the best treatment for this situation is. In other words, the general dentist wants a consultation with a specialist on the problem so that he can deliver the best care possible to the patient. Nobody wants to have a root canal, post, build-up, and crown placed on a tooth if the tooth really needs to be extracted because of a bone loss situation.
To conclude, the standard of care in the health care industry is the yardstick by which adequate or sub-standard care is measured. It is most of the time based on what the average health care provider in the same community would do under the same circumstances.
